Proof of Age ( ALL players must submit Proof of Age. Can be a copy of a birth certificate, Passport or MSYSA or US Club Soccer player card. We do not accept MSI player cards. )

Submitting Paperwork

All paperwork must be submitted prior to the start of the league (Monday - Friday from 10:00 am - 5:00 pm). This is for all teams who have already submitted their team’s application along with payment and have received an acceptance email.
During this time, teams should bring the team roster, waivers and proof of age for all players. (Registration held inside the Adventist HealthCare Fieldhouse formerly known as the Discovery Sports Center)
